Contest Details and Rules:

1. Meditec.com will give away a free Apple iPOD Shuffle on Monday, December 1, 2008.

2. This contest is open to legal U.S. residents (of the 50 United States and its Territories).

3. There is no purchase necessary to win, but you must be at least 18 years of age, and you must complete the below contest entry form online in order to be eligible to win.

4. Eligible entries must be submitted via this website entry form, by Midnight (12:00 AM Mountain Standard Time) on Sunday, November 30th, 2008.

5. A winner will be chosen by a random drawing from all eligible entries received by the above deadline. The winner will be notified by email and/or by phone.

6. Only one eligible entry per household will be accepted, based on the street address listed in this form.

7. Meditec.com will pay for standard shipping of the iPOD to the address specified in this prize entry form only.

8. The Winner is solely responsible for all applicable taxes. The retail value of the prize is listed next to the description box in the entry form below.
