Contest Details and Rules:
1. Meditec.com will give away a free eBook course of your choice (single course or a combination) with a maximum retail value of up to $799.00, on Friday, August 1st, 2008.
The prize will include the eBook training course/s on CD-ROM only.
2. This contest is open to legal U.S. residents (of the 50 United States and its Territories).
3. There is no purchase necessary to win, but you must be at least 18 years of age, and you must complete the below contest entry form online in order to be eligible to win.
4. Eligible entries must be submitted via this website entry form, by Midnight (12:00 AM Mountain Standard Time) on Thursday, July 31st 2008.
5. A winner will be chosen by a random drawing from all eligible entries received by the above deadline. The winner will be notified by email and/or by phone.
6. Only one eligible entry per household will be accepted, based on the street address listed in this form. The winner will have five (5) business days from the date of notification to decide which eBook course or combination they wish to take (with a maximum/combined retail value not to exceed $799.00). Should the winner fail to notify Meditec (fails to make a choice) within the five (5) days of notification, a Meditec.com representative will then make that choice arbitrarily, and that decision will be final.
7. Meditec.com will pay for standard shipping of the course materials to the address specified in this prize entry form only.
8. The Winner is solely responsible for all applicable taxes. The retail value of the prize shall be no more than $799.00 and Meditec.com will notify the winner of the exact retail prize value (for tax purposes) after the prize choice has been made.

10. Meditec.com will not pay any cash difference for prize selections which have a retail value of less than $799.00 in retail value. The total value of the prize shall be no more than the eBook course/s the winner chooses up to $799.00, whichever is less. There will be no cash option or substitutions allowed.
